What is All-on-4 Implant Treatment?
All-on-4 implant treatment is a method of applying a fixed prosthesis using only 4 implants in a single jaw for patients who are edentulous or have severely damaged teeth. Developed by Portuguese implantologist Prof. Dr. Paulo Maló, this revolutionary technique stands out by requiring fewer implants than traditional implant treatment, largely eliminating the need for bone grafting, and allowing you to leave the clinic with temporary teeth on the same day. At Dentaline Dental Clinic, we perform All-on-4 treatment with our experienced surgical team and the most up-to-date digital planning technologies.
Who is All-on-4 Suitable For?
All-on-4 implant treatment is ideal for the following situations:
- Completely edentulous (toothless) patients
- Patients with teeth that need extraction due to severe decay, mobility, or fractures
- Patients dissatisfied with their current full denture wanting a fixed solution
- Patients who cannot receive traditional implants due to bone loss (limited bone volume)
- Patients wanting fast results and leaving with teeth the same day
- Patients wanting to avoid the complexity of multiple implants and bone grafting
Advantages of the All-on-4 Technique
- Only 4 implants: While traditional methods require 6-8, All-on-4 needs just 4
- No bone graft needed: Angled posterior implants maximize existing bone
- Same-day teeth: Temporary fixed prosthesis fitted on the day of surgery
- Shorter treatment time: Much shorter overall compared to traditional methods
- Economical: Efficiency due to fewer implants and no bone grafting
- High success rate: 95-98% success rate reported in scientific studies
- Comfortable chewing: Fixed prosthesis allows eating like with natural teeth
Treatment Process
1. Planning Phase: A 3D cone beam computed tomography (CBCT) scan is performed. Jaw bone structure, nerve canals, and sinus anatomy are analyzed in detail. Ideal implant positions and angles are determined using digital planning software. A surgical guide is prepared.
2. Surgery Day: Under local anesthesia or sedation, existing problematic teeth are extracted. Four implants are placed in the jaw bone — 2 vertically in the front region and 2 at 30-45° angles in the posterior region. This angled placement maximizes use of existing bone and eliminates the need for bone grafting.
3. Same-Day Prosthesis: Immediately after implant placement, a pre-prepared temporary fixed prosthesis (immediate loading) is fitted. You leave our clinic with your teeth on the same day.
4. Healing Period (3-6 Months): A 3-6 month waiting period allows complete implant-bone fusion (osseointegration). During this time, you continue your normal life with your temporary prosthesis; soft foods are recommended.
5. Permanent Prosthesis: Once osseointegration is complete, digital impressions are taken and your permanent zirconium prosthesis is produced and fitted. This prosthesis looks like natural teeth and withstands chewing forces.
Materials Used
Premium brand implants (Megagen, Straumann, or equivalent) are used in All-on-4 treatment. The permanent prosthesis is made from full zirconium or zirconium-substructure composite material. All materials are CE certified and meet international standards.
Care and Long-term Success
All-on-4 prosthesis requires care similar to natural teeth. Daily brushing, special implant floss (superfloss), and professional check-ups every 6 months are recommended. A water flosser (irrigator) is helpful for cleaning under the prosthesis. With proper care, an All-on-4 prosthesis can last 15-20 years or more.
